The reason and method of establishment

St. Anne’s University Hospital Brno is a state contributory organisation established and directly managed by the Ministry of Health of the Czech Republic. St. Anne’s University Hospital in Brno provides comprehensive diagnostic, therapeutic and nursing care, institutional, outpatient and day care and to perform complex pharmacological activities. Designated workplaces of the St. Anne’s University Hospital Brno are used as teaching centres for the Faculty of Medicine, Masaryk University.

Requests for information

A request for information under Act No. 106/1999 Coll. May be submitted in writing or by telephone.
Telephone requests for information will be submitted to the Legal Affairs Division, Hybešova 42, Building H.A (tel. +420 543 183 281 or +420 543 183 282). If the applicant is not provided with the information upon a telephone request, or if the applicant considers the information provided upon a telephone request to be insufficient, a written request must be submitted.

The receipt and submission of applications

A request is considered submitted when it is received by St Anne’s University Hospital Brno (hereinafter FNUSA).
The request must clearly state that it is addressed to FNUSA and that the applicant is requesting information within the meaning of Act No. 106/1999 Coll.Individuals will provide their first name, last name, date of birth, place of permanent residence or, if they are not registered for permanent residence, their home address, and mailing address, if different from their permanent residence or home address. Legal entities will provide their name, registration number (IČ), registered office and mailing address, if different from the registered office. The mailing address also means an electronic address.

If insufficient information about the applicant under the previous paragraph prevents the application from being processed, FNUSA will ask the applicant to complete the application within 7 days of the date of submission. If the applicant fails to do so within 30 days, FNUSA will not proceed with the application.

If the request is unclear, it is not obvious what information is being requested, or it is formulated too generally, FNUSA will ask the applicant to clarify the request within 7 days of the date of submission. If the applicant fails to clarify the application within 30 days, FNUSA will decide to reject the application.

If the requested information does not fall within the scope of FNUSA’s activities, the request will not be processed with justification and FNUSA will notify the applicant thereof within 7 days of the request being submitted.

FNUSA will provide the requested information within 15 days of receiving the request or of receiving any additional requested information or clarification. The deadline for providing information may be extended due to serious reasons but by 10 days at most. The applicant will be informed of the deadline extension, including the reasons for it, in a timely and demonstrable manner.

If the request for information cannot be granted in whole or in part for legal reasons (such as due to the protection of personal data, trade secrets, protection of classified information), FNUSA will decide within 15 days of the request to reject the request or any part of it.

A written appeal may be filed against a decision to reject an application. Appeals are submitted to the same contacts as requests for information. FNUSA will forward the appeal within 15 days of receiving it to the supervisory body, i.e. the Office for Personal Data Protection, which will decide on the appeal within 15 days of receiving it.

Any complaints regarding the procedure of handling requests for information may be submitted in writing or in person (by telephone). If a complaint is made in person (by telephone) and cannot be addressed with immediately, FNUSA will make a written record of it. Complaints may be filed within the time limit and for the reasons specified in Act No. 106/1999 Coll., on Free Access to Information, as amended. Complaints will be decided by the Office for Personal Data Protection.

Licence agreements

In order for FNUSA, as an obligated entity, to provide information that is subject to copyright protection, the applicant must enter into the license agreement below, which will govern the conditions for providing such information. As an obligated entity, FNUSA must have property copyright to the required information that enables it to distribute or publish it, which allows it to provide the information without infringing on the rights of third parties (such as the author, etc.).
